Link Out Case Manager – Complex Care

  • Permanent Part Time position, based in Cranbourne
  • Provide individualised plans to assist clients transition to the community

The Australian Community Organisation (ACSO) is a non-government, not for profit agency which provides accommodation, support and assessment and treatment services for people who are experiencing significant disadvantage.

The Link Out group is responsible for providing pre and post release support to clients in the Southern Metro and Gippsland regions.

The primary purpose of this position is to assist the transition of clients back into the community in a structured and planned manner, enabling them to build their social and economic participation and reduce their recidivism.

We are seeking someone with:

  • An appropriate tertiary qualification relevant to the delivery and management of health and welfare services.
  • Demonstrated experience of at least 2 years in case managing a similar client group and understanding their needs and limitations.
  • An ability to develop positive relationships with participants, model appropriate social behaviours by communicating messages clearly and sensitively as well as setting clear boundaries around the scope of the work.
  • The ability to work as part of a team in a multi-disciplinary environment, as well as to work autonomously with a high degree of organisational skill.
  • Knowledge of the criminal justice and prison systems and an ability to work within a prison environment.
  • Knowledge of the range of health, welfare and community agencies ACSO’s programs interface with.
  • A demonstrated ability to communicate, work with, and understand the needs of the various client groups i.e. homeless, offenders, mental health consumers, addiction sufferers.
  • A demonstrated knowledge of the relevant and current legislation and policy relating to the client group as listed above.
  • Excellent administration skills including developing case plans, professional case noting, writing court reports and effective time management skills.
  • The ability to implement skill development and behaviour support plans.
  • The ability to work within a stressful environment.
  • High level interpersonal skills and an ability to communicate and negotiate, liaise and consult effectively with a range of people.
  • Current Victorian Drivers license.
  • Current First-Aid certificate (Level 2) or willing to obtain.

Forensic Disability Residential Welfare Worker – Gippsland

A forensic disability residential worker provides support for adults with a disability, complex presentations and offending behaviours. These adults are subject to varying legislative orders which may include; Supervision Orders, Supervised Treatment Orders, and court/correctional orders. The Specialist Forensic Disability Residential Worker develops implements and reviews behavioural and skills building programs to improve quality of life with the aim of assisting individuals to transition back to his/her community, reduce the risk of re-engaging in offending behaviour and provide opportunities for community reintegration. If you have a passion for connecting with adults with a complex presentations and making a difference to their lives, we want to hear from you.

These positions report to the Regional Manager Gippsland.

ACSO are seeking suitable applicants with:

  • Relevant qualifications, with a minimum requirement of Diploma in the area of Disability. Those with qualifications in Mental Health, Alcohol and Other Drugs or equivalent are encouraged to apply and will be supported to obtain qualifications in Disability.
  • Extensive relevant experience in working with people with disabilities or offending behaviours.
  • Outstanding interpersonal and communication skills.
  • The ability to work as part of a team in a multi-disciplinary environment.
  • Exemplary verbal and written communication skills.
